Chronic Disease Management Department
Hello, my name is Korin William Humchitt, I am a Heiltsuk Registered Nurse working in the Chronic Disease Management Department here at the Hailika’as Heiltsuk Health Center; and my name is Sonja Kathleen Leafloor, I am from the Snuneymuxw First Nations, and I am the LPN working in the Chronic Disease Management Department.
The Chronic Disease Management Program services those in Bella Bella that are living with Chronic Conditions; these chronic conditions can include – diabetes, cardiovascular diseases, respiratory diseases, autoimmune disorders (Arthritis), high cholesterol, and so much more.
Soon we will be providing regular foot care here at the Health Center and the Elders Building all year round. Once the Public Health Orders is lifted, and restrictions begin to ease from COVID-19, we will begin hosting regular clinics, including: Diabetic Clinics, Foot Care Clinics, Pain Management Clinics, Heart Health and Vital Signs clinics, and Kidney Check Clinics.
Our program takes a holistic approach to chronic disease management and prevention, by including other professional – Fitness Coordinators, Nutritionists, and the team of Physicians at the R.W Large Memorial Hospital. Our goal is to be consistent health care providers that help with communication, guidance, and advocacy, so you all have the best health care that you deserve.
As our Department continues to develop, and we offer more services, we will continue to update and provide information to the community Members of the Heiltsuk Nation.
